dementia - the protagonist is clearly insane, seeing himself as literal monster when he enters an adrenaline induced rage, during which he can tear his enemies limb from limb. Others do not see the monster, though. Also considering the final boss is a manifestation of his hate, the events of that fight are rather questionable. There are also other people who are more or less insane, such as Dr. Killjoy.
outlaw protagonist - possibly framed, but this isn't established solidly in the first game. The sequel seemed to expand on this.

I seem to recall that one of the "evil bastards" implied that the protagonist was responsible for all the shit going on.

The game blocks normal printscreen key usage, so taking screenshots of any kind requires 3rd party software which I personally don't have or know of any that don't add ads to the screenshots such as Fraps.
