Features sentient beings driven to some crazed state in which they may be difficult comprehend and usually are quite aggressive. Panic, fear, even some "subtle" outside influences can cause this.

This is for when you know or can make a very good guess that they aren't insane, mind controlled, or otherwise under influence of something vicarious, but more driven (temporarily) mad from fear or similar cause.
The naming of the tag kind of implies they've become somewhat aggressive. Shooting at ghosts, so to say. Unfortunately the player is usually the one in the receiving end of their madness and due to limited nature of many games, likely has the only solutions of completely avoiding them (rare) or killing them, barely ever having the possibility to incapacitate them, but such is the flaws of mad people having guns.
This also implies that they can be cured, or presumably so, though there's no telling what kind of psychological trauma remains.
For cases with truly insane people, use the lunatics tag.
