For both melee and ranged weapons. In case you get to mix melee and ranged weapons and similarly unrelated weapons, use asymmetric dual wielding instead.

Also for the unlikely cases where the character has more than two hands that it uses for wielding weapons.

Usually the weapons or tools are "activated" simultaneously or otherwise from same action button. Swinging two swords or firing two guns at the same time or in quick succession.
