Any immediate healing the player can use can not accomplish perfect results.

The wounds may eventually heal "perfectly", but this must require significant amount of time in game time terms to still qualify for this tag.

This is not for lack of healing methods that immediately restore all health, but that even repeated use of any healing methods can't reach back full health.

Essentially this discourages any activities that can harm the player character.

May imply extremely slow health regeneration to recover the portion that can't be healed artificially.

This is not when there's simply lack of healing methods that immediately restore to full health (e.g. only healing items that restore 20hp out of total 100hp), but rather that no matter how many of you take them, you won't reach back to full health with them. Nor is this where their healing effect is gradually applied.
