[b]dementia[/b] - the protagonist is clearly insane, seeing himself as literal monster when he enters an adrenaline induced rage, during which he can tear his enemies limb from limb. Others do not see the monster, though. Also considering the final boss is a manifestation of his hate, the events of that fight are rather questionable. There are also other people who are more or less insane, such as Dr. Killjoy.
