It is extremely common for characters in fiction to recover even the most ludicrous wounds, transformations, and whatever else so much that you can't tell they ever had such incident unless they're left there for story or character building. This tag therefore is for cases that do not follow this tradition.
Like the child tag for scarring, players generally do not like this being present as it stains the image of perfection and epicness they want their characters to exude.
